Need blank HPD inspection visit sheet

Hey all, Can someone scan in or take a photo and attach the inspection visit sheet that goes in the 6×9 frame that HPD requires? I just got the sheet and the frame but now the sheet has gone MIA before I could attach the thing. HPD should put a blank pdf online but naturally there isn’t one. You could cross out whatever you want or should, I just need the form so I can print one out to stick inside the frame. Thanks.

Do they always leave a sheet? My current tenants called HPD on me for the first time (Ive had this ouse for over 22 years without a single formal complaint)and they came and I got 3 violations, 1 because the hot water was too not, one for a missing smoke detector(I think because it’s supposed to be near the bedroom but not IN it and the 3rd for my egress door lock, which I’ve had replaced with a thumb turn lock, not sure this is the correct term.) I don’t see any sheet that says they’ve been there? I have a 2- family house. Any info would be very helpful and appreciated. Thanks,
