Why did he think all the plumbing lines need replacing?
Sure you can probably do some repairs here and there, but sounds like you’re ultimately going to need to replace all the stacks.
I doubt the 40K includes the new boiler; but then again, I don’t know how big the house is; and how much plumbing work is involved.
Just a thought, when you replace a plumbing stack, you’re going to have to open up the entire shaft, access the walls behind the fixtures and access through the floors. Not a pretty job. You might as well re-do the bathrooms at that point. And don’t forget patching everything up to make it look good; that’s not included in the plumbing number.

THIS is the “stack” to which the inspector was refering. If you yourself noticed that it is blocked in one place, and that water comes out in different places when you turn a valve on, the stack is blocked in many places. Better to get it replaced.
Also, the stack and water lines have nothing to do with the boiler. That is a different job.
It sounds like you have a gut rehab on your hands. I agree with DIBS that you should call in an architect.
