Wood Floor Replacement Recommendation

Conte did a very good job on replacing our kitchen floor a couple of years ago. However the original re-finished subfloor, while shabby and splintery, was not so bad that it needed to be removed; they installed new oak over 1/2″ plywood.

I have a master bedroom and guest room that need the floor and subfloor replaced due to termite damage. Who can you recommend?
