I just looked at a 1040 and there’s nothing on it for a Roth contribution. Also, briefly scanned IRS publication instructions and no mention of a special form to fill out.

right, but not sure where on 1040 you indicate than making (or made a ROTH contribution) — a regular IRA yes – but I don’t recall seeing anything about ROTH.

I don’t have a Roth, myself, ony an IRA and a 401k. I’m going to ask the Schwab people.

I think they do a more in-depth interview with randomly selected people, to gain that information.
