Co-op Management / Services Survey

Does anyone have any experience or examples of a type of survey that can be fielded annually by our co-op board to gauge residents’ general satisfaction levels? We are a 200 unit co-op dealing with the usual management and upkeep issues but there doesn’t seem to be any baseline of data or information we can use to improve upon. Would be great to get an annual or biannual read of our progress or lack thereof. Any help would be appreciated.
