Anyone able to submit the coop abatement renewal form on nyc.gov?

I’m the treasurer for a small, self-managed coop, and I’m trying to renew our coop abatement using the Co-op Tax Benefit Change Form for 2018/19 to let them know about a unit sale last year. Any time I submit the form to log in with our borough name and coop number, I get the error message:

NO RECORD EXIST FOR THIS BORO/COOP NUMBER

Is anyone able to use this form successfully? I had the same issue trying to access it online last year. I tried to get help via email and they confirmed I was submitting the right info but they weren’t able to fix my issue, and I ended up having to submit a paper change form. This year I don’t even see an option for a paper form though?
