If you want to spend about $250 a square foot for a gut reno (no demo, no joist work); would that include the fixtures? (kitchen appliances, bath vanity, etc.)

I don’t think that could include fixtures. Keep in mind that most bids dont include not only things like appliances and plumbing fixtures, but you’ll also be responsible for lighting, door hardware, sometimes windows…
