Go to a cash basis for primary care.
Leave the hospitals for the surgical specialty care and major medical at low premiums and high deductable.
Ask your doctor next time if he/she would be satisfied with $100 per patient visit (to include injections, routine lab tests and diagnostics) with no further paperwork to file.
$100 per visit is not that much considering that’s what the plumber charges you to come over and look.
