Experience with BSIU complaint? (against a licensed plumber)

Does anyone have any experience with pursuing a complaint through the Buildings Special Investigations Unit?  Unlike DCA, BSIU doesn’t seem to have a standard complaint form, and so far I haven’t found any information about the process or possible penalties.

investigation unit deals with officials corruption and on plumbing side with cover ups of unicensed plumbers. other then that you have to call Dob plumbing division, and they wil advice you.
