As much as I love original details, when I had to replace a small, badly damaged medallion in my hallway I bought this polyurethane foam reproduction which, IMO is indistinguishable from plaster after being caulked and painted:

This particular model isn’t available anymore (possibly because it was more expensive than Chinese-made poly medallions) , but there are many others.
