Electrical Inspections

Just out of curiosity, hoping for input from actual electricians if you’re out there, along with anyone else who has been through this, when the inspector comes to check your work and fails you and then a different inspector comes to the next inspection and fails you for something that you did in response to the first failure what’s really going on?
a) the first inspector was wrong
b) the second inspector was wrong
c) the electrician screwed something up
d) inspection failure quotas at work
e) differing interpretations of code
