Chimney (vent) leak and long term fix?

Need help identifying a proper fix for a leak. We have this thing on the roof edge that looks like a chimney (vent?). In some homes, there are pipes coming out on the top, in our home the top is sealed with black tar. We discovered the tar dried up and the elements ate into the brick mortar, resulting in a water leak down the wall to the 1st floor. This chimney is used for the kitchen exhaust going to the back of the house wall. We fixed the leak by applying new black tar. My guess is that fix will

We have a similar leak around the base of a plumbing vent where it meets the roof. We fixed it with flashing cement, I believe, the first time, but now that fix doesn’t seem to be working again.

We don’t know if outside of that the vertical vent inside the brick is sealed. I’m thinking of doing a kitchen reno in the 1st floor owner apartment and would like to fix this the right way during that time. The rest of the roof is in very good shape.

The tar wil likely seal off the top from water infiltration. But, more importantly, is the kitchen vent fully contained in ductwork until it exits? If not, it’s introducing moisture into the brickwork and mortar and they will eventuallly fall apart.
