Replacing Door Threshold

My main exterior door threshold has gotten worst over the course of the past couple of years. There is a noticeable gap on the left side of the threshold. Does anyone know how easy is it to replace one? is it all standard sizes or do I have to go to a carpenter? is it as easy as just taking out the screw and putting the new one in? Thanks in advance

If you are able to raise it by backing off the screws, get some shims under it to eliminate the gap.  Also, some weatherstripping at the bottom of the door also helps.
