All it is that DOB did not approve for the LNO due to either contradictory information or due to lack of information which is usually detailed on the LNO application being returned to the applicant and they want an application be filed to document the use.
All other items are not relevant and are just old files. DOF designation doesn’t mean anything for DOB and since I-cards are not obtainable for less then 3 families this is a very common problem of there is no block and lot folder.
Not much can be told by that. You need to talk to an architect. It might be that the zoning does not support a two family or it might be whatever they filed to make it a two family was not properly drawn up and filed.
