    do you remember what happened at 3 mile Island?
    It came down to human error.

    when the reactors heated up,
    the computers opened the water gates
    to flood the reactor rods and cool them down.

    the personnel at the plant saw the gates opening
    and panic ensued.
    they did a manual override and closed down the
    gates that would have provided the necessary cooling
    water.

    it derailed our energy independence for generations.

    …this story is just beginning.

    I still don’t have a full handle on this story,
    but basically the well was “blown out” by the fire and
    blast on the rig above water.
    Engineers had the option to stop it up before things got worse.
    A decision was made to use sea water instead of heavy sediment to plug the hole (presumably with the expectation of unplugging it for easy access after things were stabilized).
    That decision to use seawater allowed the pressure to mount and the crap hit the fan.
