Take Care Central Brooklyn NYC

In 2004, the Health Department created a citywide health policy called to help improve the health of New Yorkers. TCNY identifies 10 key areas that cause significant illness and death but can be improved through intervention by individuals, health care providers, government agencies, and other organizations. This report examines how well Central Brooklyn residents are doing on health indicators for each of the 10 TCNY goals. It examines areas in which the community is a health leader, as well as areas that need improvement.
