Call Manny Lassalle (917) 292-1825. He has them occasionally, restores and installs them. I bought a magnificent summer-cover insert from him last spring. I’m sure he can get you what you want.
If you decide to go with rh’s slate mantle, you’ll need an artist who specializes in faux painting. It will look great.
Salvage places sell them. Many have Web pages even if most of their inventory is not online, and they ship. It’s hard to find the kind of slate fireplace pictured above, though. Most seem to focus on wood mantles.

I have a slate one leaning against my garage that I intended to repaint and sell. It was originally done in a faux marble, but it really does need to be redone. It’s all faded. You can make me an offer on that if you want. I’d deliver.
