Minimum Sill Height

I am about to replace windows in basement. Basement is not finished, but I would not mind to do it at some point.
So firstofall I noticed code, which said that min height of the basement sill should be 6″. Does it mean that there should be 6″ from the ground to the window sill? Is it bottom side of the sill?

Also should I put frosted glass? Sounds like a good idea but I do not see many frosted basement windows around.

Finally should I remove the iron bars? Should I leave them?
