Tenant Application Form and credit check

I’m looking for a standard Legal Tenant Application Form to download, buy, or get for free. I will use it to screen potential tenants for 1 apartment in my 2 family home. The form must have a credit check autorization area.

Any recomendations for the tenant application form will be much appreciated. Seeking also, Credit Check Websites recomendations for Landlords.
