How to close out open construction permit?

Earlier this year, our architect filed a permit for our modest renovation. When the job was complete, inspection was supposed to be done “self-cert.”
The architect has long since disappeared, and now that the job is nearing completion, I’m wondering how do we close out the job with NYC and the appropriate agencies?
